On 24th and 25th of October 2019, the kick-off meeting for WP4 of VALUE-Dx was hosted at the Julius Centre of Health Sciences and Primary Care of the UMC Utrecht in Zeist, the Netherlands.

Representatives from different community care settings, from the industry partners and from other work packages came together in preparation for the point prevalence audit survey (PPAS) which is planned to take place during the ‘high-peak’ winter season (January-March 2020) in 21 countries across Europe. The PPAS aims at describing the presentation and management of community-acquired acute respiratory tract infections (CA-ARTI). Furthermore, this PPAS is the first step for WP4 in designing the clinical trial testing various point-of-care diagnostics for CA-ARTI in community settings to better target antibiotic use and ultimately combat antimicrobial resistance.

Key highlights

• To build a European community care research network, the existing primary care network from PREPARE is being extended with eastern European countries and combined with research networks for A&E care in hospitals (COMBACTE), pediatric care (Penta) and long-term care (LOTTA-Net).
• Representatives from primary care networks from as many as 18 countries introduced their network and highlighted key differences with respect to health care delivery in their country, epidemiology of RTI and resistance, and the management of and available diagnostic testing for RTI. Partners from COMBACTE, Penta and LOTTA-Net shared insights in their networks and (research) experiences.
• The meeting featured a highly interactive session on methods and logistics of the PPAS to ensure flexibility and relevance to the different countries involved and meeting the needs of the four community settings.
• The industry partners bioMérieux, Abbott and Becton Dickinson showcased their point-of-care diagnostic products to be potentially included in the clinical trial.
• To enhance proper integration of all involved the evening started with a pub quiz with general questions from all over Europe, European song contest winners, and a section with VALUE-Dx related questions.

On 28 September to 2 October 2019, the European Respiratory Society (ERS) held its annual International Congress of 2019 in Madrid, Spain. Bringing together almost 23,000 delegates from around the world for a packed week of scientific and educational sessions around respiratory medicine, ERS put on a lively display of clinical presentations at the capital of Spain.

On 29 September 2019, a symposium was held on ‘Fighting antimicrobial resistance: from prevention to antimicrobial stewardship’ that was chaired by WP6 co-lead Prof. Murat Akova of ESCMID and Dr. Eva Polverino of ERS. Project lead of VALUE-Dx, Prof. Dr. Herman Goossens, gave a compelling talk on ‘The new era of diagnostics: will it ever start?’, presenting the evolving market of in vitro diagnostics as the world’s largest med-tech sector. Despite the booming interest, MedTech Europe reported that diagnostics only represent 0.8% of total healthcare expenditure in Europe in 2016.

When it comes to antimicrobial resistance, diagnostics such as point-of-care-testing would be instrumental in fighting this global problem. With approximately 80% of antibiotics prescribed in primary care and over-prescription a real concern, diagnostics would pave the way towards proper use of antibiotics in primary care settings. In the future, better and quicker diagnostic tools should be able to transform the current practice of empiric antibiotic therapy into a directed and precise antibiotic therapy, therefore improving clinical outcomes of patients with infections.

This summary provides more information on the future work that VALUE-Dx will carry out in the coming years across 20 European countries on community-acquired acute respiratory tract infections (CA-ARTI). This includes details the three winter seasons from 2019 to 2022:

• 1st winter season of 2019-2020 – a web-based point prevalence audit survey (PPAS);
• 2nd winter season of 2020-2021 – platform randomised controlled trial of point of care diagnostics (PRUDENCE trial);
• 3rd winter season of 2021-2022 – PRUDENCE trial.

Whilst the web-based PPAS will record information about patients seeking healthcare for CA-ARTI, the PRUDENCE trial will evaluate clinical algorithms that incorporate a point of care test for CA-ARTI.

Furthermore, this summary provides an update on the activities that have already begun during the first year of VALUE-Dx.

On 27 September 2019, the first WP2 face-to-face meeting took place in Antwerp. With 15 attendees, all partners involved in WP2 were well represented. After some updates on the WP2 division of work and building of the External Advisory Panel, every partner presented their capabilities to successfully carry out their tasks. University of Antwerp described their expertise on the different areas required in this WP, such as biomarker research, next generation sequencing techniques, bioinformatic tools and biobanking activities including the set-up of the Antwerp biobank that is ready to receive samples from WP4. The status of WP4, on which the tasks of WP2 heavily depend, was discussed at length. Bioaster described their involvement in many projects and some novel bioinformatics tools and ideas on how to perform their tasks, which they are already field-testing. bioMérieux provided information on how they could  support the rest of partners in  WP2 and their vision on the  development of the VALUE-Dx biobank. This first meeting yielded fruitful discussions in which clear synergies were identified among the partners.  These activities will be discussed further in the upcoming monthly telecons of WP2.

The 10^th Advanced Course on Diagnostics organised by Mérieux Foundation and the London School of Hygiene & Tropical Medicine was held at Annecy, France, from the 15^th to the 20^th of September. A week-long intensive course that brought together professional participants from across 35 countries, the focus of this course was on the role and the value of diagnostics in global health for antimicrobial resistance control.

Prof. Dr. Herman Goossens, Coordinator and Project Leader of VALUE-Dx, gave a talk on ‘Setting the scene for public-private collaborations in diagnostics’ for the session on ‘Role and value of diagnostics in Global Health’. Prof. Dr. Goossens’s presentation delved into the partnership that has been set up within the VALUE-Dx consortium due to its unique collaboration between industry and non-industry partners. In addition, his presentation touched on the current landscape of diagnostics to combat antimicrobial resistance and the added advantage that VALUE-Dx will bring to demonstrating the socio-economic value of diagnostics.

With sustainability being at the core of VALUE-Dx to ensure the continuation of its infrastructure, VALUE-Dx’s sustainability plan was presented to include the building of the biobank, database, and the infrastructure to carry out clinical trials on diagnostics.
